Use this form to certify that a child between 18 and 23 is attending school full-time, which allows them to continue as a dependent for VA benefits purposes.

How to Complete This Form
Complete this form for each child 18-23 attending school:
**Section I - Veteran/Claimant Information**
Your identifying information.
**Section II - Student Information**
- Student's full name and SSN
- Date of birth
- Relationship to veteran
**Section III - School Information**
- Name and address of school
- Type of school (college, vocational, etc.)
- Date enrollment began
- Expected graduation date
- Whether student is full-time
- Course of study
**Section IV - Previous Education**
List any previous schools attended after age 18.
**Section V - Certification**
Both the veteran and student should sign.
