This form is used for specific SMC claims:
**Section I - Veteran Information**
Provide your identifying information and VA file number.
**Section II - Type of SMC Claimed**
Indicate which type of SMC you're claiming:
- Loss or loss of use of a hand or foot
- Blindness or deafness
- Loss of reproductive organ
- Need for regular aid and attendance
- Housebound status

Helpful Tips
- SMC is additional compensation on top of your regular disability rating
- Different SMC levels (K, L, M, N, O, P, R, S) have different requirements
- SMC-K is commonly awarded for loss of use of a creative organ
- SMC-S (Housebound) requires one 100% disability plus additional 60%
- Submit medical evidence clearly showing the qualifying condition
