Use this form to appoint a Veterans Service Organization (VSO) such as the American Legion, VFW, DAV, or others to represent you in your VA claims. VSOs provide free assistance with claims.

How to Complete This Form
Complete the form to appoint your chosen VSO:
**Section I - Veteran Information**
Your personal and contact information.
**Section II - Claimant Information**
If someone other than the veteran is filing (surviving spouse, etc.).
**Section III - Organization Selection**
Select the VSO you want to represent you. Common options include:
- American Legion
- Veterans of Foreign Wars (VFW)
- Disabled American Veterans (DAV)
- AMVETS
- State VSOs
- Many others
**Section IV - Authorization**
- Authorize VSO to access your VA records
- Authorize VSO to change your address (optional)
- Authorize VSO to act on your behalf
**Section V - Signature**
Sign and date the form.
**Note:** You can only have one VSO representative at a time. Filing a new 21-22 revokes any previous appointment.

- You can switch VSOs at any time by filing a new 21-22
- Consider choosing a VSO with local office for in-person help
