How to Complete This Form
A Supplemental Claim allows you to submit new and relevant evidence to have your claim reconsidered.
1. Complete your personal information in Section I
2. In Section II, identify the issue(s) you want reviewed
3. Describe the new evidence you're submitting or want VA to obtain
4. You may request a VA medical exam if appropriate
5. Sign and date the form
New and relevant evidence must relate to your claim and not have been previously considered by VA.
Helpful Tips
- Make sure your evidence is truly NEW (not previously submitted) and RELEVANT (relates to the reason for denial)
- Request a new medical exam if your condition has worsened
- Include a nexus letter if that was the reason for denial
- Consider getting a private medical opinion if appropriate
Common Mistakes to Avoid
- Submitting evidence that was already considered
- Evidence that doesn't address the reason for denial
- Not explaining how the new evidence relates to your claim
- Filing without actually having new evidence
Deadline Information
No deadline for filing, but you cannot file a Supplemental Claim for the same issue more than once within one year unless you have new and relevant evidence.
