How to Complete This Form
A Higher-Level Review (HLR) is appropriate when you believe VA made an error in applying the law or evaluating existing evidence.
1. Complete your personal information
2. Identify the specific issue(s) and decision date
3. Optionally, request an informal conference to discuss your case
4. You may identify specific errors you want the reviewer to consider
5. Sign and date the form
No new evidence can be submitted with an HLR. The senior reviewer will only look at evidence already in your file.
